About the Role:

In this role, you ll be joining an integrated health and social care service dedicated to supporting individuals independence and enhancing their quality of life. The service focuses on community rehabilitation and reablement, helping people recover from injury, illness, or significant life changes. You ll work within a multidisciplinary team of health and social care professionals, all united by the goal of restoring and maximising independence while preventing deterioration in physical, psychological, and social functioning.

Key Responsibilities:

  • Deliver Impactful Care: Assess, plan, and implement occupational therapy interventions that support your clients' rehabilitation and reablement goals.
  • Collaborative Environment: Work closely with a multidisciplinary team to create and execute personalised care plans tailored to each individual s needs.
  • Expert Support: Provide specialist advice and guidance to clients, their families, and other healthcare professionals involved in their care.
  • Continuous Improvement: Monitor and evaluate client progress, adapting care plans to ensure the best possible outcomes.
  • Empower Independence: Help clients regain their independence and improve their quality of life through targeted, effective therapeutic interventions.
  • Professional Standards: Maintain accurate, up-to-date records in compliance with professional standards and organisational policies.

Qualifications and Experience:

  • Must be HCPC Registered: A current registration with the Health and Care Professions Council (HCPC) is essential.
  • Degree in Occupational Therapy.
  • Experience working with individuals with disabilities, ideally within a community or rehabilitation setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team.
  • A strong commitment to delivering high-quality care and a genuine passion for supporting people s independence.
