About the Role:

Intermediate Care and Service is looking for a full time Experienced Qualified Social worker to work a 5-day week inclusive of the weekends.

The candidate will join a dynamic integrated team with Adult Social Care and NHS Community based team for adults and older people. Team provides an Urgent Response service to prevent hospital admissions working collaboratively with social care staff, therapists, and nurses to ensure that our statutory responsibilities are being met.

There is some flexibility with the weekend working pattern and can be negotiated if appointed as successful.

You will provide high quality statutory social work services to adults with care and support needs and their families in a fast-paced environment to ensure that they maintain living as independently and as safely in the community as possible.

To be shortlisted for an interview, you will:

  • Have a relevant social work qualification and must be registered with (Social Work England)
  • Have experience of working in a fast pace team such as a hospital discharge team or an urgent response team
  • Have experience of working in a multi-disciplinary team
  • Have compassion and an understanding of the needs of adults with care and support needs and their families
  • Have excellent working knowledge and understanding of the relevant legislation, guidance, and practices
  • Be an excellent communicator
  • Must have a minimum of 2 years post qualifying UK experience working with Adults, adults with disabilities or older adults
  • It is essential that in your written supporting statement you give evidence or examples of your proven experience in each of the short-listing criteria marked Application (A)
  • Must provide answers to all mandatory criteria in the Person Specification.
