Job description

Personal Advisor Children and Families
1x Mansfield, 1x Ollerton (support children and young people across Nottinghamshire as well as some who live out of county area)
Umbrella Pay- £18.25 per hour
10/11/2024 3 months initially
Monday to Friday, 8.30am 5pm, 37 hrs/week Hybrid

  • Direct work with care leavers from the age of 16, managing a caseload
  • Ensure young people are prepared for adulthood transitions
  • Contribute to the assessment process and implementation of Pathway Planning
  • Review the needs of care leavers to ensure that plans and services reflect current needs and risks
  • Coordinate how services are provided for the young person
  • Stay in regular contact with the young people, noting their progress and wellbeing
  • Deliver positive outcomes by offering direct support, advice, guidance, practical help and emotional support
  • Support with housing issues, managing money, health and life skills
  • Identify situations that require the input of a qualified worker and to seek advice and guidance when needed
  • Work with a range of partner organisations to assess, plan and intervene with vulnerable young people
  • Ensure the views of the service users and their families are taken into account
  • Immediately raise any concerns with may have
  • Develop strong, trusting working relationships
  • Be a proactive and positive member of the team
  • Participate in individual supervision with the line manager, as well as undertaking appropriate training and developmental opportunities
  • Keep and maintain up to date records
  • Attend meetings, reviews and conferences where necessary
Requirements
  • UK Driving Licence, access to a vehicle, business insurance
  • Recent experience working directly with Young People leaving care
  • Knowledge of the statutory functions of the local authority in relation to care leavers and
  • Ability to organise, prepare and run support groups
  • Ability to work pro-actively in the attainment of good outcomes for children in care and care leavers
  • Good written and verbal communication skills
  • Enhanced Child DBS registered onto the update service
