The The Senior Community Health Worker is a trusted member of their community who acts as a liaison between community and the health system. A Senior CHW acts collaboratively with other members of the care team while demonstrating responsive communication with diverse populations. The Senior CHW has experience providing culturally appropriate education and can support community members and patients as they navigate complex social and health systems. Health Worker III (CHWIII) promotes comprehensive health and well-being within the community they reside in. CHWs are an integral member of patient-centered health teams.
