Freedom Plaza Care Center is a Skilled Nursing Facility located in Peoria, Arizona and has been serving the community for nearly 30 years. We are currently seeking an Occupational Therapist to join our skilled therapy team. NEW GRADS ENCOURAGED TO APPLY Work schedule will include normal weekday scheduling with rotating weekends. Full benefits are offered with full-time employment.

Duties/Requirements:
Appropriate degree in Occupational Therapy from an accredited university
Current state license
Current CPR
Demonstrate ability to evaluate patients with physician referral.
Develop Plan of Care and treatment plans to maximize patient potential.
Provide adequate daily documentation on patient treatments.
Supervise Occupational Therapy Assistants
Communicate patient needs and physical functioning to supervisor and interdisciplinary team.
Participate in discharge planning including placement, patient and family education, and adaptive equipment.
Initiate family training and nursing mobility programs to keep patients moving outside of therapy sessions.
Experience preferred but not required.

Minimum Qualifications
Current state licensure or eligibility for state licensure as an Occupational Therapist required.
Certified BLS or completion in the first 90 days of employment required.
Fingerprint clearance card
