SUMMARY
A team lead is responsible for implementing workflow procedures based on the direction from the Health Tech Navigator's supervisor and team manager. Duties include providing key mentorship to health tech navigators, serve as subject matter expert on patient outreach and provide day-to-day agent support.
ESSENTIAL FUNCTIONS
- 70% of time will be spent on direct enrollments
- Enrollment/Navigation goal will be adjusted accordingly
- 30% of time will be spent on administrative program duties (i.e. 1:1 check ins, troubleshooting, meetings, team coordination, and special assigned projects)
- Changes in percentages of time will be communicated to leads based on program needs
Team Lead duties and responsibilities
- Schedule Health Tech Navigator team meetings.
- Attend meetings on time and be present and prepared.
- Schedule Team Lead meetings.
- Review key metrics (example: enrollment, navigation) of assigned navigators weekly.
- Plan and coordinate 1:1 team check in with assigned staff:
- Weekly for new staff
- Biweekly for existing staff ( > 3 months on-site employment)
- Prepare agenda and slides for weekly Team huddle meetings.
- Lead weekly Monday Team huddle meetings.
- Help prepare weekly Team email: send shoutout email on Thursday or Friday to supervisor.
- Manage, update, and maintain Team Handbook (One Note).
- Analyze site data and efficiently present to relevant staff to create a system of improvement at your location.
- Keep records of all 1:1 meeting, write weekly and bi-weekly summaries of 1:1 meeting, review these weekly in Team Lead meeting with program supervisor and assistant manager.
- Provide coaching and support to assigned navigators, especially those who are performing below individual goals.
- Attend and be actively engage in Team Lead Check In meetings with assistant manager and/or program supervisor.
- Provide program feedback from navigators to program supervisor and assistant manager during weekly Team Lead check in
- Present ideas to increase enrollment and the use of the LA Health Portal on Sites during weekly Team Lead check in meetings.
- Check in to see if navigators have marketing materials and if not escalate to program leadership.
- Provide ongoing support to other team members:
- Provide day-to-day Teams chat troubleshooting
- One lead to be scheduled daily
- Perform site visits as directed by program supervisor.
- Develop expertise in the LAHP features and capabilities.
- Attend and participate in LAHP Champion meetings.
- Serve as a team example of navigator leadership and professional communication skills with navigators, program supervisor, assistant manager, site champions, site leadership, and others.
- Work on special projects as assigned by the program supervisor and assistant manager:
- Example: written patient guides, patient videos, training materials, job aids
- Direct all questions about the program to assistant manager.
- Be prepared in meetings related to portal outreach and engagement.
General navigator duties and responsibilities
- Update navigation log daily.
- Update personal outlook calendar with daily activities and assignments:
- Share calendar with program supervisor and assistant manager.
- Communicate issues that may happen on site with program supervisor.
- Be on time to site and other meetings you are asked to attend.
JOB QUALIFICATIONS
Education/Experience
- Bachelor's Degree preferred or 5 years of experience in related field
- Excellent verbal communication, and the ability to convey important information clearly and effectively
