FHCSD's Program for All-inclusive Care for the Elderly (PACE) is designed to help our patients maintain a high standard of independent living. Our program provides a wide range of supportive services including in-home care, meals, medical equipment, transportation, social services, and social activities. We are currently seeking an Occupational Therapist to provide care at our PACE - Oceanside location.

In this position you will work as part of an integrated health team assigned to each patient. The Occupational Therapist is responsible for facilitating the development and rehabilitation of PACE participants with mental, emotional, and physical disabilities by planning and administering medically prescribed occupational therapy. Our goal with every patient is to create an individualized program that helps restore their physical function, enhance emotional and social well-being, alleviate pain, and prevent physical dysfunction.

Position Duties

Assess, plan, prepare, and carry out individually designed programs
Maintain, improve or restore patients physical functions
Enhance emotional and social well-being
Evaluate the effects of treatment at various stages
Adjust treatments to achieve maximum benefit
Work with participants to meet their goals and needs.
Assist PACE participants to develop or regain physical or mental functioning
Help participants adjust to disabilities
Implement programs involving manual arts and crafts
Facilitate practicing prevocational, vocational and homemaking skills
Design home exercise programs
Recommend and/or provide assistive devices
Instruct patients, families, and caregivers on in-home exercise programs
May oversee staff that assist in the care of the PACE participant.
Provide information to the participant about the proposed intervention, possible risks and expected benefits, and any reasonable alternatives.
Collect participant data and complete required forms to ensure compliance with regulatory requirements.
Attend meetings and actively participate in the care planning
Perform other duties as assigned

Requirements:

Current California state licensure as an Occupational Therapist.
Bachelor's degree in Occupational therapy; Master's degree in Occupational Therapy preferred.
Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
Ability to meet FHCSD Driving Requirements (if position requires).

Desired Experience

1-2 years of experience working with the older adult population.
Clinical competency with the older adult population is preferred.
Knowledge of appropriate and updated therapy equipment and technology to include aerobic and strength conditioning equipment tailored to the older adult population and use of mechanical assist devices.
Ability to educate other staff on proper body mechanics.
Ability to work effectively in a team environment.
Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on participant status and needs
Bilingual English/Spanish preferred.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
