Job description
Patient Navigator The Bilingual Patient Navigator links immigrant, refugee, and non-English speaking patients with health, psychosocial, and other services to using a culturally sensitive, patient centered approach. This position collaborates with internal and external partners to guide patients through the nuances of the US healthcare system. ESSENTIAL FUNCTIONS Provide comprehensive health navigation including assistance with: Health insurance enrollment and verification Registration and appointment check-in PCP assignment Appointment scheduling, confirmation, and reminders Interpretation and completion of forms and paperwork Directing patients to internal departments based on their needs Working with clinical teams to review and ensure understanding of treatment plans Interpreting messages between the care team and patient Coordinate with external immigrant and refugee services, creating partnerships to help identify and connect patients with OCH services Deliver patient education in the form of printed materials, group health workshops, telephonically, and verbally during visits to the health center Assist with outreach telephone calls to reduce care gaps and keep patients engaged in care Remain current on health-related community resources and act as a point of contact, following up on resource needs and troubleshooting patient barriers to accessing services when necessary Assist with translation of patient facing signage, documents, and educational material in collaboration with the communications department Assist with producing training for staff that promotes cultural awareness and sensitivity Represent OCH at community events, providing information about services in multiple languages Additional Duties Attend all scheduled staff meetings Work collaboratively with all members of the care team and extended medical team Active participation in team huddles Active participation in quality improvement initiatives Active participation in multidisciplinary committees and workgroups Assist with the training of new staff in collaboration with leadership and preceptors/mentors Coverage of PSR and eligibility duties as needed based on staffing availability All other duties as assigned MINIMUM REQUIREMENTS High School Diploma or GED At least 2 years of customer service experience At least 1 year experience working with diverse populations including refugees and immigrants Fluency as well as the ability to read and write in Dari, Pashto, and English Understanding and sensitivity to the cultural nuances and practices of Afghan communities, particularly regarding healthcare beliefs and practices The ability to problem solve independently as well as the ability to collaborate in a team setting to work toward a specific goal Excellent interpersonal and communication skills Current BLS certificate PREFERRED BACKGROUND Prior experience working in healthcare settings, preferably in a role involving patient interaction or navigation Existing knowledge of external, local resources and services for immigrants and refugees Degree or certification in relevant field such has healthcare administration, interpretation, or linguistics Reasonable Accommodations One Community Health endorses and supports the Americans with Disabilities Act of 1990 (ADA) and the California Fair Employment and Housing Act (FEHA) and is committed to providing reasonable accommodations to qualified individuals with disabilities who are applicants or employees who need accommodations.
