J ob Summary The Home Visitation Health Navigator will work fully out of the Get Enrollment Moving (GEM) office and focuses on identifying members in the community in need of support through home visitations and referrals to appropriate home visitation agencies. This position will work closely with agencies in SPA3 (San Gabriel Valley) that provide home visitation services to establish a referral partnership with GEM Medi-Cal assistance services. The HVHN will contribute to a supportive infrastructure for pregnant individuals or young children in need of extra support. Functions as both an educator for home visitation programs and contact for participants enrolling in Medi-Cal and/or "troubleshoot" an existing applications, utilization and renewal services. Job Requirements Minimum Education Requirement : High School diploma or equivalent and Associate's Degree preferred. Bilingual/bicultural in English and Spanish. Knowledgeable of computer programs. Will need to complete the CORE comprehensive training certification and Home Visitation Orientation. Minimum Experience Requirement : One (1) year of experience with screening, enrollment, retention and trouble-shooting of health care benefit programs for low-income communities preferred. Ability to establish supportive and trusting relations in an ethnically diverse population with empathy, compassion and confidentiality. Excellent customer service skills required. Minimum License Requirement : Completion of the Maternal Child Health Access "We've Got You Covered" training within 45 days of hire required. Certified Application Counselor (CAC) Certification required or completion of Covered California training based on availability. Valid California Driver's License and proof of insurance required.
