Job Description

The RN Case Manager reports directly to a Clinical Supervisor and is responsible for care coordination of clinical staff, patients and their families/caregivers, and physicians or other care providers. Responsible for admissions, recertification assessments, transfers/resumptions of care, and discharge assessments, including supporting documentation.

Duties and Responsibilities

Ensures direct supervision of CNAs, LPNs, and RNs in the field. This involves training and re-education of staff on clinical skills/procedures when necessary.
Develops an individualized plan of care for each client.
Provides support, guidance, and education to patients and their families/caregivers to assist patients in their progress toward their goals.
Completes all clinical documentation in a timely and accurate manner, and ensures all documentation meets the requirements of agency policies and state regulations.
Complies with and participates in continuing quality improvement by reporting incidents, injuries, and infections.

Qualifications

Current state license as a Registered Nurse, BSN preferred.
Minimum of one years of nursing experience required (two years preferred).
Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
Detail oriented, flexible, good organizational and time management skills.
Current CPR card and Current health certificate as applicable.
Current state driver's license and automobile insurance.

Additional Information

The salary range for this position is $70,000-$75,000 annually. You are also eligible for a monthly auto and cell phone allowance totaling $4,200.00 annually.
