About the Role:
As a Registered Nurse at Poudre Home Care, you will:
Be the case manager for every client you open. Provide and direct the care prescribed by the physician or other primary care provider.
Work in a highly collaborative environment, coordinating your plan with other skilled clinicians, paraprofessionals, agency personnel, physicians, midlevel provides, and community resources to make a difference in the lives of clients.
Make comprehensive evaluation visits and, in collaboration with the team, develop a skilled nursing plan of care for the client.
Follow up on your plan to provide skilled nursing care and high-quality education that clients and their caregivers need to stay safe and to progress to their provider-approved goals.
Expect variety! We do not insert IVs or PICC lines, but we manage PICC infusions at home, teach clients to perform infusions, perform skilled wound assessments and care, provide the crucial hands-on management of changing treatments and medications, educate clients in their new diagnoses, perform -and much more.
We offer a full orientation and preceptor program to new staff: you won't be sent to a challenging case without the training you need to succeed.
What We're Looking For:
Experience and Education:
New RN grads welcome! We offer a new residency opportunity to recent RN grads.
Must possess a nursing degree from an accredited university.
Clear, unencumbered licenses form the State of Colorado as an RN.
1-2 years of related experience or training preferred.
Preferred experience in the geriatric population in a long-term care facility, assisted living, or other related health care facility.
Skills:
Ability to effectively able to read and interpret documents.
Ability to communicate effectively.
Basic mathematical skills.
Ability to reason and problem solve.
Ability to lead and supervise nursing assistants for the assigned unit.
Basic technical skills.
Knowledge of the aging process, dementia, and disabilities.

Our Total Rewards package includes a salary range of between $35 - $45/hr. (DOE), in addition to:
Opportunity to earn up to $150 or more for admitting new clients!
Pay-per-visit fee structure which provides flexibility while rewarding hard work.
