Certified Occupational Therapist Assistant - COTA

Summary:

We are looking for a committed and patient-focused Per Diem COTA - Certified Occupational Therapist Assistant to work for our home care department. The COTA will be responsible for providing therapy services under the supervision of a Registered Occupational Therapist and whose license is in good standing, to patients in their homes, in accordance wtih the written plan of care establised by a physician. You will offer effective and client-centered services that will enable people to carry out everyday tasks or activities confidently and independently.

This is a field-based position.

Responsibilities:

Provide occupational therapy to patients by assessing and understanding the care plan.
Evaluate patient's functional status and home environment for hazards or barriers to be independent living and identify any equipment needs.
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.

Supervised by an Occupational Therapist.
Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
Submits all clinical documentation, time records and other information in the required time frame.
Supports company mission by providing the highest quality of home care.

Qualifications:

Licensed Certified Occupational Therapy Assistant (COTA) in the state of Connecticut with at least 1 year of clinical experience; Home Care experience preferred.
Oasis experience strongly desired.
Have a valid driver's license, reliable transportation and willing to travel throughout an assigned territory.
Ability to work independently with minimal supervision.

Ability to solve problems.
Have great interpersonal skills.
Excellent computer skills.
