Job Title: Rehabilitation Supervisor
Department: HomeCare
Hours: 400
Shift: 1st (8a-4:30p)
Weekends Required: No

Job Summary: The Rehab Supervisor will function as the therapeutic resource and supervisory person for the therapeutic staff. The Rehab Supervisor will be responsible for the coordination of quality patient care and for the appropriate utilization of resources in the Homecare Rehabilitation area.

ESSENTIAL RESPONSIBILITIES

Evaluates, treats and records/maintains complete accurate patient record on Portable Electronic Device and/or paper record as prescribed by referring physician, while educating patient/caregiver.

Is accountable for maintaining established productivity goals, continuity of care, and appropriate utilization of resources. Performs concurrent and retrospective audits, participates in quality improvement by serving on a QI team or cross-functional team, and participates in the development and analysis of Outcome Measurements and Outcome Management.

Participates in the process of interviewing, hiring, evaluating and disciplinary action. Teaches and evaluates personnel in areas of referred instruction.

Uses clinical thinking skills and special medical knowledge to care for the sick, acutely ill, physically challenged, injured, or handicapped individuals.
Understands technical information from supervisors, charts, reference books, manuals, and labels.
Uses eyes, hands, and fingers with skill.
Works expediently and accurately in emergency situations.
Communicates well with patients and family when they are ill, injured, handicapped or apprehensive.
Demonstrates flexibility within the job environment.
Identifies and responds to priorities to ensure quality patient care and effective staff utilization.
Maintain discretion in handling the confidential nature of the information available.
Keeps up to date on current trends and developments in the field of physical medicine and health services.
Qualifications:
Specific vocational preparation includes an occupationally significant combination of: education, training, and on-the-job training.
A minimum of (3) years of experience in a clinical practice setting and one year of clinical Physical Medicine and five years related professional experience required.

Education:
Minimum: BS degree in Physical Therapy, or Occupational, or MS or MA degree and a Certificate of Clinical Competence) for Speech Therapy required.
CT license or CT license eligible required.
CPR Certification required.
