Job description

Must have DC License in hand
Social Worker/Discharge Planner - 40 hrs Days
Shifts: 8am - 4:30pmDay and rotating weekends

Will accept RN Discharge Planner or Licensed Social Worker
Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already

Certs Required : BLS,RN or LMSW/LGSW/LCSW

Work Setting skills Req : Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing
Work Setting skills Preferred: Home Health, Hospice, telephonic, previous charge experience

Case Management/Utilization Review skills required : Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker's comp case management

Regulatory Skills Required: CMS: Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission/ Core Measure/National Safety Goals
Regulatory Skills Preferred: Worker's Compensation

Any Special Requirements: Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters

Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU,

Holidays - as needed
Weekends - Every 5-6 weekends must work one shift)
Cerner is required
Uniform: Business Casual-Associate's appearance should be clean, neat and professional. NO SCRUBS
Does not accept 1st timers

Job Type: Full-time

Pay: $55.00 - $65.00 per hour
