Carlton Shores Health and Rehabilitation Center is looking for Registered Physical Therapists (RPT) to join our dedicated team of caregivers!

Summary of the Registered Physical Therapists (RPT):

The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Essential Duties & Responsibilities of the Registered Physical Therapist (RPT):

Creates, implements, and executes patient treatment plans.
Communicates with patients, families, referring physicians, and other members of the team.
Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
Develops and enhances clinical and professional skills through knowledge and professional associations.
Supports and participates in department operations and development.
Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.

Job Requirements of Registered Physical Therapist (RPT):

Current Florida License as a PT (Physical Therapist)

Current CPR

At least one (1) experience working in a Skilled Nursing Facility preferred.
