Job description
We seek a nephrology nurse practitioner or physician assistant to help care for patients with acute or chronic kidney/urologic disorders, those undergoing dialysis, and those who are candidates for or have undergone kidney transplants. Our ideal candidate would have advanced knowledge and experience in nephrology, including pathophysiology and management of kidney/urologic conditions and their complications. As part of an interdisciplinary care team, this position will provide telehealth and onsite visits as part of ongoing medical management and acute interventions for patients in the post-acute care setting. DUTIES AND RESPONSIBILITIES may include but not be limited to
- Provide collaborative care and coordination of dialysis patients.
- Conduct patient assessments, Health Risk Assessments (HRA) and Comprehensive Disease Assessments (CDA) through telehealth and onsite patient visits.
- Manage dialysis treatment and medication, and provide patient education and support.
- Responsible for documentation and record-keeping.
- Participate in quality improvement and comprehensive assessment.
- Provide coverage as needed in other areas of nephrology including clinics, infusions, and procedures.
- Coordinate the patient care plans, address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals.
ESSENTIAL ABILITIES
- A solid foundation of nephrology, renal care issues, standard nursing practice, and treatment options available to patients with chronic kidney failure.
- Ability to provide both telehealth and onsite visits to skilled nursing facilities.
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each time
- Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
QUALIFICATIONS
- Master's degree in Nursing Licensure, Certification, and Registration
- Current, valid Florida APRN license Certification through the ANCC, AANP (or equivalent certification body)
- Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
- No weekends or evenings work is available at this time.
- 3 or more years of clinical experience in nephrology, including dialysis experience.
- Geriatrics and telehealth experience are highly preferred.

Our clinician-focused technology and resources will help you:
- Conduct clinical assessments and evaluations.
- Formulate treatment plans as needed.
- Prescribe medications with proper dosage and administration instructions.
- Oversee telemedicine patient flow and daily functions of the telemedicine shift.
- Maintain accurate electronic medical records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.
- Maintain integrity, availability, and confidentiality of all PHI
- Oversee the reporting of diagnostic testing results, follow-up treatment, referrals, and recovery.
- Participate in quality assurance and care improvement initiatives.
- Adhere to HIPAA standards for information security, privacy, and confidentiality.
