Position Summary

The Home Visit Specialist is responsible for assisting LCHC with managing their health and for coordinating ongoing outreach activities within LCHC's target communities. The Home Visit Specialist will manage a case load of patients (pregnant patients, infants or other high-risk patient groups) providing regular home visits, and aid patients in overcoming barriers to good health outcomes. Time between home visiting and community outreach will be divided according to program needs. The Home Visit Specialist reports to the Care Coordinator Manager.

Principal Duties and Responsibilities

Care Management and Community Outreach

  • Schedules and completes home visits with assigned caseload, meeting face-to-face expectations as identified by the Care Coordinator Manager.
  • Completes community outreach as requested by care team care management staff to engage or provide specialized support to high risk or hard-to-engage patients.
  • Completes care management tasks and documentation as necessary.
  • Connect patient with appropriate support, coordinating linkages and resources as appropriate.
  • Assist patients in obtaining WIC, health insurance, and other social support services as needed.
  • Facilitate ongoing communication with the patient's primary care provider and care team care management staff to ensure comprehensive and coordinated care.

Documentation and Accountability
  • Appropriately document all home visits and phone encounters in LCHC's EMR system.
  • Participates in regular supervision and staff meetings.
  • Responsible for performing other duties as required to assist LCHC in achieving its mission.

Required Skills or Abilities
  • Demonstrates excellent communication skills.
  • Requires experience and demonstrated abilities for working in a multi-cultural setting.
  • Ability to strongly embrace and personify the mission of the Lawndale Christian Health Center, through performing work in a whole person way, providing for the physical, emotional, and spiritual needs of staff members and patients.
  • Ability to collaborate and interact with a diverse group of health care professionals.
  • Ability to organize and prioritize work appropriately.
  • Excellent consumer relations skills.

Required Knowledge, Experience, or Licensure/Registration
  • Bachelor's degree in a health-related area or an associate degree with two years' experience in providing direct professional child, family, or community services.
  • Ability to speak Spanish a plus.
  • Valid driver's license and insured automobile.
