Outpatient/Home Health PT Role and Responsibilities:

Demonstrates knowledge of home health and competency in discipline specific patient care skills such as activities of daily living, energy conservation, UE strengthening, and pain management.

Performs home health physical therapy services in accordance with physician's orders and agency policies.

Performs patient comprehensive assessments, as assigned, and collaborates with patient multi-disciplinary care team (SN, OT, ST, MSW and paraprofessionals) and other health care professionals to develop/modify and implement an individualized plan of care as per physician orders that ensures quality, proper discharge planning and achieves desired outcomes and goals.

Supervise, educate and collaborate with licensed Physical Therapy Assistants and Home Health Aides.

Participates in clinical outcomes monitoring, follow up and Quality Assurance Performance Improvement initiatives.

Utilizes accepted company teaching materials in patient/family education.

Regularly communicates patient progress to the clinical leadership and the patient's physician.

Completes documentation timely, accurately, and at the point of care, according to industry standards, and guidelines.

Demonstrates knowledge of physical therapy in an outpatient setting and competency in discipline specific patient care skills such as medication management, chronic disease management and education, wound care, gait training, functional mobility, ADL, balance and coordination etc.

Completes documentation timely, accurately, and at the point of care, according to industry standards, and guidelines, including OASIS assessments, SOC/Admissions, Recertifications, ABN's, subsequent visits, physician orders, care coordination, etc.

Required:

Current Illinois license to practice as a Physical Therapist.

One (1+) year of clinical experience as a Physical Therapist.

Current CPR certification.

Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online training programs.
