Purpose:
Mental Health DSP will participate as a member of the multidisciplinary team responsible for the supervision, training and implementation of an individual's treatment plan and behavior management plans. Facilitate all client activities, and complete documentation as required by the multidisciplinary team, CARF Accreditation and funders including Medicaid standards.
Essential Responsibilities:
Provide compassionate care and recovery-oriented services to clients with mental illness.
Provide input into the client's treatment plan by outlining strengths, challenges and accomplishments.
Assist clients to achieve independent living goals by providing training in key areas such as activities of daily living, budgeting, transportation, developing healthy relationships and other items defined by ITP goals.
Assist in the development and adherence to a healthy menu (based on client's dietary needs) and in the preparation of meals as indicated by the client's skill level and treatment plan.
Provide and supervise recreational activities with clients in the community.
Supervise clients in the self-administration of medications. Provide training to them on the process, usage and side effects. Follow up with supervisor and/or medical professionals around any issues or concerns that happen related to medications. Some clients may require staff to administer the medications in which DSP's are trained to provide.
Document medication use as required by DHS standards for each shift worked.
Read and initial the communication log at the beginning of each shift.
Document services to clients as outlined in their ITP on individual or group note forms. Notes must be compliant with agency standards and completed before the end of the shift.
Complete other documentation including shift notes, attendance logs, communication and medication logs, incident reports and contact notes as applicable.
Assure safe, clean and secure living arrangement for all clients by assisting them to complete household cleaning chores. Complete the tasks as necessary by the end of the shift.
Aid the program in maintaining regulatory compliance standards by conducting fire, tornado and evacuation drills

EDUCATION, SKILLS, and/or EXPERIENCE:
- Be at least 21 years old
- High School Diploma or GED
- Able to pass a criminal background check
- Must have a valid Driver's license
- 1 year of social service experience, preferred
Open Positions: North of Chicago
- FT: Sunday, Monday & Tuesday 2pm-12am
- PT: Saturday & Sunday 9am-3pm
Compensation: $16.93 per hour.
