Shift: Days / 8a-5p, 8x5, 40 hours per week, Call required and weekends - every 3rd to 4th weekend as needed

Length: 13 weeks

Requirements:

• IL License
• 1 year home care experience
• Minimum 1-2 years MS or acute care experience
• Ortho experience
• OASIS Doc Experience
• BCLS
• Phlebotomy Cert

Other Details:

• Special Requirements: clinical nursing, PT, OT in home
• Point Of Care visits: wound care, wound vac, IV therapy (antibiotics, TPN), post ortho/joint, incision assessment follow up, post surgical assessment/complications, post stroke, pneumonia; patient education meds, wound care; trach care, teaching; GTubes feedings, teaching; ADLs assessment, OASIS Doc Exp. wound vac, glucometer, pro-time machine PT/INR, IV pumps, central lines / ports, need ortho experience, Customer Service and excellence
• Territory/County covering: Territory mileage radius: 40-80 miles.
• Ratios: 5-20 visits/day average
