Unit Rate: $65.00 per unit
Mileage rate $0.55 a mile
Start of Care Visit (SOC) 2 Units - $130 Plus Mileage

Physical Therapist - Home Healthcare
As a Physical Therapist (PT) in home healthcare with AssureHHI, you shall provide services that help restore function, improve mobility, relieve pain, and prevent/minimize permanent physical disabilities of patients suffering from injuries or disease. A successful Physical Therapist teaches and trains patients and caregivers to be an active participant in their recovery to restore, maintain, and/or promote overall fitness and health. Their patients include accident victims and individuals with other disabling conditions such as low back pain, arthritis, heart disease, fractures, hip and knee replacements, head injuries, strokes, and other neurodegenerative diseases and gait disorders.
Responsibilities:
Provide skilled physical therapy in accordance with the physician's plan of treatment.
Assist the physician in the evaluation and assessment of the client's level of functioning by applying appropriate tests and assisting in the development and revision of the plan of care.
Treat clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.
Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the physician, Clinical Manager, and Case Manager.
Educate client/caregiver and health care team in the phases of physical therapy in which they may assist the client, in the use of prosthetic and orthotic devices, and durable medical equipment, as appropriate. Provide staff in-service education as directed by the Clinical Manager.
Assess and regularly reevaluate the needs of the client. Develops implements and revises the client's plan of care.
Initiate diagnostic, preventive, and rehabilitative procedures as appropriate to the client's care safety.
Assist with a plan of care and maintain continuity of and/or multidisciplinary clients whenever needed to ensure good coordination of care.
Promote coordination of care through interdisciplinary case conferences and communication with physicians and field staff.
Develop, prepare, and maintain individualized client care progress records with accuracy, timeliness, and according to agency policy and procedures.
Participate in the agency's quality improvement program.
Keep abreast of physical trends and knowledge for service provision, documentation, and care coordination.
Qualifications:
Must be a licensed Physical Therapist according to the standards established by the Commission on Accreditation in Physical Therapy Education (CAPTE).
At least 2 years of experience as a physical therapist, some experience in home health or public health nursing preferred.
Proficient in utilizing technologies including Microsoft Word, Excel, PowerPoint, Outlook, Teams, SharePoint, and other software related to role and responsibilities.
Hold a valid driver's license, own a dependable vehicle, and maintain a clean driving record.
