SUMMARY AND OBJECTIVE:

The Licensed Practical Nurse (LPN) is a key member of the nursing team and is responsible for maintaining the activities and functions of resident care and nursing services. The Licensed Practical Nurse is responsible for providing quality care and quality services to the residents of the facility through solid clinical skills and the use of other caregivers and support staff. The LPN will review staffing to determine that sufficient caregiving staff with the appropriate competencies and skills are available to provide resident safety and attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident, as determined by resident assessments and individual plans of care and considering the number, acuity and diagnoses of the resident. The LPN follows clinical systems, infection control, quality assurance, policies and procedures to assure that residents receive care that is consistent and reliable. The Licensed Practical Nurse monitors, manages, and directs the clinical standards for each resident to maximize each individuals quality of care and quality of life while honoring resident choice.

REQUIRED EDUCATION AND EXPERIENCE:

Licensed as a Licensed Practical Nurse in the state of Kentucky.
Minimum work experience of two years.
Experience in long term care, gerontology, and/or rehabilitation.
