Job description

Under the direction of the Program Director, the Case Manager is responsible for providing Cause-Driven management and implementation of transitional housing services and wrap-around services. The focus of transitional housing services is to support homeless families as they acquire the tools needed to gain sustainable permanent housing. The Case Manager will support the case management of 12 families through their 24-month matriculation in transitional programming providing structure and wraparound service support. The Case mange will also maintain a caseload of families, and develop and implement life skills training and support in the implementations of all areas of the program. ESSENTIAL FUNCTIONS: Work with associates to develop systems to ensure consistent, high-quality outcomes, and services for clients; Ensure that the highest quality crisis intervention, life skills, education, counseling, and housing services are provided to all clients based on both internal and funder requirements; Works directly with a caseload of clients, providing high-quality case management services and referrals; Ensure that all documentation, including HMIS requirements, case files, client statistics, and data entry are accurate, up to date, and in compliance with grant requirements; conduct periodic audits of case files and supportive documentation; Manage program according to grant/contract specifications; Work together with the Program Director and client to develop Individual Service Plans; Conduct assessments with the client to determine goals, barriers, and resource needs; Ensure enforcement of program housing rules and regulations; Attend and participate in staff meetings and agency training; Prepare for and coordinate site visits and files reviews by funders and/or monitors both internally and externally; Assist in the development and implementation of Continuous Quality Assurance activities; Incorporate the four character values of the YMCA: Caring, Honesty, Respect, and Responsibility and adhere to the YCM Code of Conduct Communicate the Y mission and objectives to the community. Assist in all other areas as assigned. Assist in facilitating groups and presentations;
