Rockville, Maryland The salary range for this position is $70,000 - $75,000 annualized based on skills and experience. Job Summary: Implementation, coordination and supervision of all aspects of a comprehensive Health, Wellness and Clinical Program which addresses the needs of all Assisted Living Residents in accordance with Victory Housing, Inc. Policies and Procedures, COMAR 10.07.14 and Board of Nursing Regulations and Standards. Oversight includes: Provide nursing assessments, care planning, care oversight and quality assurance for all residents. Maintain all charts to include, progress notes, identification of nursing interventions, treatments and assessments. Implement a Wellness Program to include weekly scheduled resident Health and Wellness Check-ups, to assure a proactive and preventative clinical approach. Oversight and coordination of all third party vendors to include but not limited to Podiatrists, Dentists, Lab, Pharmacy, Home Health, Hospice etc. Fulfil the role of Clinical Leader in the community as an essential manager, resident advocate, case manager, delegating and practicing RN. Areas of Responsibilities: Ensure that the attainment of Victory Housing's Mission Statement takes precedence over all decisions and actions. Assume the responsibilities of the Case Manager/Delegating Nurse as defined in the COMAR 10.07.14 Regulations. Responsible for all aspects of Pharmacy and Medication Management. Responsible for the monthly change-over of medication cards; accuracy of monthly medication delivery from Order to MAR; training staff on new medications, parameters, orders and special conditions; as well as updating the Emergency Sheet (electronically and hard copy). Critical areas of responsibility are In conjunction with the Assistant Director, responsibilities include interviewing, hiring, mentoring, training, evaluating and managing each Medication Technician. Each Medication Technician is working under the license of the Health/Wellness/Delegating RN. Oversight and management of medication administration must be in adherence to the Nurse Practice Act as well as Federal, State and local regulations. Writing in all orders from physicians into the POS/MAR according to VHI policy. Making daily/weekly checks of MAR (EMR/Hard Copy) to ensure proper administration and documentation. Maintain overall quality of the resident care program in accordance with all local, state and federal regulations and the highest standards of nursing practice. Work closely with all staff as RN on issues. Proactively intervene whenever a status change is assessed. Implement a comprehensive Health and Wellness Program designed to proactively address physical, psychosocial and rehabilitative needs of the residents. Include quality assurance and outcome measures which monitor trends and anticipate potential problem areas. Schedule weekly Check-ups with each resident to monitor all aspects of the resident's condition and intervene as indicated. Review Medications, Fall Risk, Vital Signs, Weights, Nutritional Status, Skin Assessments and as well as overall Clinical, Psychological and Social Status Changes. Record information on Wellness Assessment Forms, in Progress Notes (weekly) and Community Log (as needed). Maintain regular communication with resident, family (in absence of ED) and physician on individual health care issues. When a status change is identified, be the primary liaison with healthcare professional including pharmacy, lab and Hospice etc. Actively participate in monthly care planning meetings, Quality Assurance Meetings, Family Clinical Reviews, and Hospice meetings etc. Actively participate in RSD. Report all resident status changes, follow-up actions and next steps. Educate as needed. Participate in Monthly Staff Meetings and provide In-service Training as requested by ED. Coordinate (in conjunction with ED) dietary needs with FSD and environmental needs with the Maintenance Tech. Assist the ED/AD with identifying and resolving employee concerns, complaints, grievances and issues. Assists the ED in completing 90-day and annual performance reviews. When requested by ED, will assist in counseling caregivers and medication technician. Available 24/7 to support clinical needs of the Residents. Direct the community programs for infection control, quality assurance, outcomes monitoring and other oversight programs. Review all resident charts on a regular basis, checking for content, clarity, completeness and appropriateness of care. Responsible for the quality of all resident charts, including compliance with applicable regulations and the highest standards of nursing practice. Instruct caregiving staff on clinical and nursing issues. Assess each prospective Resident prior to move-in. In partnership with ED, recommend appropriateness of placement and level of care. On day of move-in or within 24 hours, meet with the resident to discuss: Wellness Check-ups, establish baseline, start clinical chart and review/establish care needs and schedule in coordination with ED/AD/RSD. Complete clinical components and follow-up of resident individual service plans with the Executive Director and the caregiving staff. Complete reassessments and service plans for residents as needed. Responsible for completing all assessments as mandated via COMAR Regulations. Coordinate services of outside agencies providing health care or other assistance to residents, including home health agencies, physical therapists, hospice, laboratories, durable medical equipment suppliers and other professional services. Review and sign-off on Incident Reports with the Executive Director and Assistant Director. Establish intervention and documentation of follow-up. Attend classes, seminars and meetings. Maintain a high level of knowledge regarding current nursing theory, trends and regulatory changes. Scheduled hours are from 10:00 - 6:30 or 9:00 - 5:30 based on community preference. The schedule must be set and adhered to consistently. In the absence of Executive Director and Assistant Director, the Health/Wellness/ Delegating Nurse is responsible for the day to day operations of the community. Other duties as assigned. Requirements: Must be current on all vaccinations including COVID- 19 and receive CDC recommended booster doses. Job Qualifications : Must be a graduate of an accredited school of nursing with three years experience in nursing, preferably in geriatric services. Must hold a current Maryland license as a registered professional nurse. Baccalaureate degree in nursing or related health field with geriatric emphasis preferred. Must have successfully completed the 20 Hour Delegating Nurse Course within 60 days of employ.
