Description of Position

Be a part of this innovative program that is the first of its kind in Massachusetts. CCBC and the Department of Mental Health (DMH) will work collaboratively in the PACT-Y program to meet the needs of youth and families who want a creative, flexible, dynamic team to support them in achieving their goals. A Family Support Partner has personal experience as a parent/caregiver to youth with mental health and/or substance use challenges. The Family Support Partner will help to establish and maintain engagement of youth and their families, as a person with their own lived experience, who can provide unique support, guidance and mentorship.

Responsibilities Include
  • Work as part of a multi-disciplinary team to provide the caregiver perspective and ensure that treatment goals support family-collaboration principles
  • Teach advocacy skills and provide education on how to identify natural supports, advocate for youth within educational systems and obtain resources necessary to improve family functioning
  • Share experiences of raising a child with emotional and behavioral health needs
  • Work in the home and community to provide coaching and support to caregivers
  • Coordinate care with the family, team, providers and DMH
  • Complete required documentation per program guidelines
Requirements
  • Experience as a caregiver of youth with mental health and/or co-occurring substance use challenges
  • High school diploma or GED and a minimum of 2 years working with children/families or
  • Associates degree in a human service field and 1 year of experience working with children/families or
  • Bachelor's degree in the human services field and 1 year of experience working with children and families
