MGB-16807-LPN Home Health - Newton, MA - 13 Weeks Contract

Sigma Healthcare is looking for a Home Health LPN to work at our client's faclity in Newton, MA.

Shift: 08:00 - 04:30
Responsibilities :

• Provide skilled nursing care to patients in their homes.
• Administer medications and treatments according to the physician's orders.
• Observe, assess, and document patients' condition and notify the appropriate healthcare professionals of any significant changes.
• Develop and implement a care plan in collaboration with the healthcare team.
• Educate patients and their families on disease management, self-care, and medication administration.
• Monitor and evaluate the effectiveness of the care provided.
• Ensure compliance with regulatory standards and policies.
• Maintain accurate and complete medical records.
• Collaborate with other healthcare professionals to coordinate services and facilitate the smooth transition of care.
• Provide emotional support and compassionate care to patients and their families.

Requirements :

• Must be a licensed practical nurse (LPN) or licensed vocational nurse (LVN) with a valid license in the state of practice.
• Graduation from an accredited practical nursing program.
• Prior experience in home health or a related field is preferred.
• Excellent clinical and critical thinking skills.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Knowledge of medical terminology and patient care procedures.
• Basic computer skills for documentation and communication purposes.
• Physical stamina and the ability to lift and move patients when necessary.
• Valid driver's license and reliable transportation as travel to patients' homes may be required.
