Job Description: Hospice Certified Nursing Assistant (CNA) at Elara Caring Monday through Friday/8:00 am to 5:00 pm/ Monroe As a Hospice CNA with Elara Caring, you'll provide essential personal care, assist with health-related services, and perform additional tasks to support our clients. A Day in the Life of a Hospice CNA A Hospice Certified Nursing Assistant (CNA) provides compassionate personal care and support to patients, working under the direction of a Registered Nurse (RN) to ensure a safe, respectful, and dignified environment. Daily responsibilities include: Patient Preparation and Communication: Reviews care plans and checks in with patients daily before visits. Introduces self, explains tasks, and ensures clear, respectful communication with patients, families, and staff. Direct Patient Care: Assists patients with personal care, mobility, and comfort, including safe transfer techniques using equipment such as Hoyer lifts, wheelchairs, and gait belts. Helps with nutrition, meal prep, and fluid intake as directed. Monitoring and Reporting: Observes and reports any significant changes in the patient's condition to the RN promptly, including suggested updates to the care plan within 24 hours. Emergency Response: Reacts calmly and appropriately to common emergencies (e.g., burns, seizures, bleeding) and uses skills to support patient safety in urgent situations. Supportive Communication and Coordination: Maintains positive and supportive communication with the hospice care team, patients, families, and external contacts to promote continuity and quality of care. Documentation and Privacy Compliance: Keeps accurate records of care and follows HIPAA guidelines to protect patient privacy. This role involves travel, requires participation in an on-call weekend rotation (once every other month), and demands a compassionate, self-motivated approach to working with individuals in end-of-life care. Requirements: Education: High school diploma or GED. Experience: At least 1 year of experience as a Hospice Aide or Nursing Assistant. Training: Completion of a state-approved Hospice Aide training program, meeting requirements under 42 CFR 484.36, along with a competency evaluation; CNA licensure IS required Transportation: Valid driver's license, reliable transportation, and the ability to travel up to 50% of the time. Schedule: Monday through Friday, with rotating weekend shifts required. Weekend shifts will be assigned on a rotating basis, every 6 to 7 weeks, to provide continuous support and coverage.
