Hospice LPN

Hillcrest Health and Living is looking for a full-time LPN to join our Nebraska Hospice team! Hillcrest Hospice Care is a team who works together, loves what they do, and are committed to serving those in need by providing holistic, compassionate and skilled end of life care.

The Hospice LPN provides direct patient care services with the ability to manage patient care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention.

What does a day in the life look like?
• Assess the physical, functional, psychosocial and nutritional status of assigned patients in a systematic and continuous manner to provide required patient services.
• Professionally consults with physicians, other team members, patients/families, and other caregivers to formulate a plan of care and coordinate delivery of services.
• Provides nursing services in accordance with the plan of care and utilizing the nursing processes.
• Prepares clinical documentation for bi-weekly interdisciplinary meetings to reflect patient's current status. Informs the physician and other IDT members of changes in the patient's conditions and communicates recommended interventions.
• Participates in service line meetings/team conferences to ensure the coordination of all patient/family services and prioritization of their needs are being met as required.

What should I have to be considered?
• Prior hospice or home health care experience preferred.
• LPN licensed in the state of Nebraska or compact state required.
• Three years' experience as a nurse required.
• Valid state driver's license, current automobile insurance and reliable.
• Average to above average computer skills.
• Must be compassionate and able to work independently.
