Job Title: Care Connector
Location: Omaha, NE 68124.

Welfare to Work Customer Service Training GED and/or High School Diploma Required Experience in a call center, hospital secretary, patient outreach, customer service, or medical claims office Required 1 Years

Principal Accountabilities:
Support daily operations of the integrated care management program, interventions within their scope of practice.
Performs in a Contact Center environment, effectively processing calls from Members, Providers and other internal/external areas.
Identify members with care gaps/HEDIS related health conditions and assist them in accessing care through Plan benefits/community resources.
Under the direction of Clinical Staff, will provide members with educational materials and carry out strategies to increase health care adherence and reduce barriers to care.

Key Competencies/Success Factors:
Effective, oral and written communication skills Excellent Customer Service and interpersonal skills on phone and in-person.
Proficient PC skills in Windows based environment, word processing, spreadsheets, working in database programs.
Ability to handle multiple tasks simultaneously Ability to complete accurate and timely record keeping and documentation Knowledgeable about Plan Benefits/Services Familiar with medical terminology Effective listening skills
