Provides input to evaluation of Physical Therapy Assistants
Administers to clients, according to physician's orders, various evaluations and therapeutic procedures and maintains agency records on clients.
JOB DUTIES/KNOWLEDGE
Provides skilled physical therapy in accordance with the physician's plan of care. Accepts only clients referred by the Intake, Rehabilitation Program Manager or designee. Demonstrates competency in the skills required by the agency on at least an annual basis. Is expected to average 4.5 - 5 client visits per day.
Assists the physician in assessing the client's functional level by applying appropriate tests and assists in the development and revision of the plan of care.
Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
Performs venipuncture, wound care and staple removal services when appropriately trained.
Observes, records and reports the client's reaction to treatment and any change in the client's condition to the physician and/or case manager and other staff involved.
Instructs and advises clients, family and other health team personnel including, when appropriate, certified nursing assistants, in the phases of physical therapy in which they may assist the client; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate.
Cooperates and consults with appropriate staff to provide staff education when requested and arranged by the Rehabilitation Manager.
Evaluates, and regularly reevaluates, the rehabilitation needs of the client; initiates, develops, implements and makes necessary revisions of the client's plan of care. Assesses the client's continual care needs. Assesses client's pain level and implements any modalities approved by the physician to help to control the client's pain level. Ensures that the plan of care addresses all problems identified in the assessment or demonstrates rationale for not doing so.
Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the client's care and safety. Makes referrals to other disciplines as indicated by the needs of the client or documents rationale for not doing so.
Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed or as changes in the client condition require. Participates in the discharge planning process with the client, client's family, physician and any other disciplines involved in the case.
Assists with the coordination of the total plan of care and maintains continuity of client care by collaborating with other health professionals assigned to the same clients. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients when required or helpful to ensure good coordination of care.
Supervises and teaches the certified nursing assistant assigned to same clients. Assesses the client's need for unskilled care every two weeks and revises the plan of care as appropriate. Communicates plan of care changes to the aide and scheduler as often as necessary.
Develops, prepares & maintains individualized client care progress records with accuracy, timeliness & according to policies.
Participates in the agency's quality improvement program.
Keeps abreast of physical therapy trends and knowledge for service provision, documentation and care coordination. Participates in in-service programs.
Properly applies Universal Precautions per agency policies and procedures.
Demonstrates an understanding of Medicare Conditions of Participation, Medicaid and other reimbursement regulations.
Consistently assigns clients to Physical Therapy Assistant staff. Supervises and instructs Physical Therapy Assistants who are assigned to clients. Educates the PTA on the plan of care. Expected to supervise a minimum of 3-4 clients per week with PTA staff, as long as PTA staff have visit availability. Performs monthly re-evaluations of the client to update plan of care with PTA. Also performs team conferences with PTA on client's progress at least every two weeks or a often as dictated by the client's condition.
