The Facility - Dzilth-Na-O-Dith-Hle Medical Center, Bloomfield, NM

The DZHC is located in the community of Dzilth-Na-O-Dith-Hle, NM, approximately 30 miles south of Bloomfield NM and is under the administrative oversight of Shiprock Service Unit. The facility opened in 1984 to serve the community of Herfano, Nageezi, and Counselor Chapters and the Client beneficiaries residing in the urban towns of Bloomfield, Aztec, and Farmington, NM. The DZHC provides ambulatory and urgent care services through the following departments: dental, optometry, pharmacy, laboratory, radiology, mental health, medical social services, and public health nursing. The specialty services include internal medicine, women's health, obstetrics, and diabetes education and are available by appointments. The non-clinical services are Administration, Medical Records, Acquisitions, Business Office, Facility Maintenance, General Services, Housekeeping and Security.
RESPONSIBILITIES (Included, but not limited to)
- Shall perform Internal Medicine outpatient physician duties and manage patient's needs as described and as directed by a supervisor, if applicable.
- Shall provide medical services or direct patient care services under the terms of this contract, appropriate and timely medical services in accordance with the standards of care.
- Shall provide professional Family Medicine services in direct patient care services, such as:
- Shall tend to the care of infants, children, adults and geriatric patients with acutes or chronic medical illnesses.
- Shall review clinical records and adhere to records documentation procedures to ensure that records are properly maintained.
REQUIREMENTS
- Must have an MD/DO degree, must be either ABFM certified or eligible.
- Must have a permanent, full and unrestricted license to practice medicine in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Must have a Federal DEA number.
- Must have current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS); Pediatric Life Support (PALS) is required.
- Must provide immunization documentation.
- Must have never been censured by a medical board or accused of opioid use or misuse, or sexual misconduct, or have ever been terminated early from a Locums Assignment due to behavior or quality of care concerns.
- Shall be respectful of the local, Native American, and Alaska Native culture.
This contract is currently being offered with flexibility to allow for short term (13 weeks) and long term (1-year) durations.
