Job Summary:
The LPN provides high quality care to each assigned patient/resident, providing routine daily nursing care and service in accordance with the resident/patient's assessment and plan of care. Salary range: $28.25 - $32.80 hourly. up to $3,000 sign on BONUS

Essential Duties and Responsibilities:
Demonstrates knowledge of age specific developmental factors specific to the adult and geriatric population in the planning and delivery of care
Demonstrates knowledge, skills and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease, wound care, end of life care, if applicable
Assures completion of a comprehensive nursing evaluation of a resident's physical, emotional and mental status upon admission/re-admission and when indicated, as well as, appropriate follow-up as indicated by the evaluation.
Attends various facility meetings (QAPI, morning report, care plan, wound, etc) as invited or required
Attends all required in-services
Receives report from previous shift, makes rounds and assesses the needs of the residents/patients
Reports to oncoming shift staff nurse/unit coordinator/RN Supervisor any pertinent information regarding residents
Ensures that provider (MD, NP, PA) is informed of any change in the resident's condition, abnormal lab results, abnormal diagnostic test results, consultant recommendations, etc.
Notifies provider when resident visits are due per regulations/facility policy
Ensures that provider orders are transcribed accurately and completely, and that medication and lab work are ordered if needed
Completes quality assurance audits are directed by the DON
Arranges transportation to and from outside appointments
Prepare accident/incident reports and detects/corrects situations that have a high risk of causing injury to residents; develops and implements a prevention plan for all incidents occurring on the unit
Ensures that appropriate information is sent with resident upon transfer, including advanced directives
Ensures coordination of care across disciplines
Displays optimism fostering a spirit of teamwork, positive attitude and conflict resolution among co-workers. Interacts positively with residents, family members and visitors in all circumstances
Interacts with insurance companies to ensure that benefits are provided as indicated
Maintains resident rights
Maintains HIPAA privacy, security and confidentiality standards
Follows all established policies and procedures
Maintains Corporate Compliance standards
Contributes to facility Quality Assurance Performance Improvement initiatives
Follows safety procedures by keeping areas free from hazardous conditions
Is economical and careful with supplies and equipment
Completes any other duties as assigned by Supervisor
