Job description
Overview:
Works with Site Manager to ensure meaningful community related opportunities for the people we support as well as ensure all required documentation is appropriately completed and maintained. Transports people supported to and from program site as well as to volunteer job opportunity locations or other community outreach-based activities. Responsible for program transportation coordination and oversight (including establishing routes, schedules, and maintaining associated documentation); responsible for fleet coordination and oversight (including tracking,communicating with agency fleet management and maintaining associated documentation).
Responsibilities:
Primary duties and responsibilities include but not limited to:
Ensure safety of program participants while at program site and during community-based activities
Trains program participants to perform functional/meaningful and/or volunteer job tasks to the best of their abilities
Assumes responsibility of overseeing overall functioning of site in the absence of Site Manager including supervising DSP's
Works with Site Manager scheduling all program activities and volunteer opportunities
Works with Site Manager to ensure completion and maintenance of all regulatory paperwork
Trains program participants to perform functional/meaningful and/or volunteer job tasks to the best of their abilities and to interact appropriately with peers and with persons in the community
Assists Site Manager in maintaining communication with parents/correspondents, advocates and Care Managers of program participants regarding progress, problems and all pertinent information that needs to be disseminated
Ensures protection of all participants from neglect/abuse during program hours. Reports incidents to administration in a timely fashion
Promotes and reinforces all agency philosophies including the guiding principles
Required to successfully complete and maintain all mandated trainings/certifications as determined by the needs of program volunteers/site
Establish all program transport routes and schedules including making all updates and changes as needed
Maintaining and ensuring the distribution of all transportation routes, schedules and updates to responsible parties
Communication liaison for all transportation information/issues between administration, families, departments and programs
Oversee all fleet tracking and inventory- including service of vehicles and proper maintenance of keys
Ensures and coordinates follow-up on all fleet issues (including accident reports, inspections and registrations, permits, traffic violations and tickets, vehicle violation summonses, etc.)
Performs additional duties as requested by supervisor
Provides coverage at other locations as needed
Personal outcome measures' training is required prior to completion of evaluations and assessments
Qualifications:
High School Diploma or Equivalent
Good oral and written communication skills.
A qualified NYS Driver's license and needs to be and remain a qualified driver
AMAP/First Aid/CPR/SCIP-R Training preferred
Minimum 6 months experience working with people with intellectual and other developmental disabilities but 1 year is preferred
Physical Aspects: May include, but are not limited to, lifting and transferring of an adult volunteer.
