Job description

The Discharge Planning Manager plays a crucial role in overseeing the professional practice of case managers, ensuring that departmental policies and procedures are followed, and monitoring staff performance. As an educator, implementation team leader, internal consultant, facilitator, role model, and change agent, this position is responsible for delivering quality, compassionate, culturally competent, effective, and efficient discharge planning services in compliance with professional standards, including those of the Joint Commission and regulatory requirements.

Under the guidance of the Director, the Discharge Planning Manager is responsible for the management of the daily operation of the Discharge Planning Department.

Qualification:

New York State Registered Nurse

Bachelor's in nursing degree required

Masters prepared preferred

5 or more years' experience in a case management/ discharge planning position

Certification in Case Management preferred, required within 6 months of employment

PRI/Screen preferred

Knowledge of service delivery systems

Knowledge of professional Nursing standards

Knowledge of health care reimbursement and managed care policies and requirements

Effective management/ leadership skills necessary to manage, coordinate and evaluate the activities of case managers.

Analytical skills necessary for problem solving and clinical decision-making

Excellent written and verbal communication skills

Excellent collaboration skills

Ability to manage multiple priorities
