Seeking an MDS Coordinator/Assistant Director of Nursing to join the team of a skilled nursing facility located in Oswego, NY. This role is ideal for an MDS Coordinator who is looking to get into a more clinical role as an ADON.

Willing to train on all ADON responsibilites

Salary: $85K to $90K (based on experience)

Qualifications:

• Must have New York RN License
• Must have MDS experience in long term care
• Must have strong leadership skills and experience
• Must know MDS 3.0

Job Description:

• Conduct and coordinate the development of the resident assessment (MDS)
• Maintain and periodically update written policies and procedures that implement MDS and care plan.
• Assist the resident in completing the care plan portion of the resident's discharge plan.
• Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents.
• Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings.
• Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews.
• Assist in supervision of nursing staff in the facility
• Ensuring there is adequate nursing staff, and that the staff's skills remain current
• Overseeing nursing employee conduct
• Being knowledgeable of incidents at the facility
• Assessing the health needs of each resident
