Job description
Sign on Bonus - $10,000, Paid annually over 5 years.
Wage Range - $34.73-$42.02+/Hour, More based on education and experience
Position Type/Expected Hours Of Work -40 hours per week, flexible to the needs of the program. Contact time in all program sites and Article 16 Clinic is required.
Qualifications- Bachelors, Masters or Doctorate degree in Physical Therapy. Graduation from an accredited program of Physical Therapy. Certification to practice in New York State as evidenced by current licensure as a Physical Therapist by the New York State Education Department. Work may require lifting, pushing, or pulling, in excess of 30 pounds.
Work Authorization/Security Clearance -Authorized to work in the United States.
Summary/Objective - Adhere to professional, ethical and legal standards of practice as well as all agency specific policies and procedures. Serve individuals of any age with a variety of congenital, developmental, neuromuscular, skeletal or acquired disorders/diseases through the Early Intervention Program, Preschool Program, Day Habilitation Programs, or Residential Programs as indicated. Perform and document an initial exam, evaluating the data to identify problems and determine intervention using appropriate tests, skilled professional observation and supplementary information from other agencies and records. Develop, implement, modify, and maintain plans of care and treatment goals/desired outcomes based off of the initial exam and subsequent re-evaluations. Ensure all paperwork requirements are adhered to in a timely manner. Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. Document therapeutic interventions in a manner that is compliant with industry standards and that will achieve reimbursement from our funding sources. Attend and participate in meetings for people on assigned caseload to act as a professional resource and advocate for the individuals being served.

Conduct assessments/evaluations, develop the appropriate treatment plan or program to address the individuals needs, establish the goals or desired outcomes from the services, provide the subsequent services to designated individuals, and report on patient progress.
Instruct staff members and family in treatment procedures to be continued at home, school, or at Day Habilitation.
Consult with medical practitioners, service coordinators and appropriate others to plan, implement and assess interventions.
Participate with the service/treatment coordinators and other team members to develop Individualized Family Service Plans, Individualized Education Programs, Individual Program Plans or Individual Service Plans as appropriate for the individual.
Complete all documentation in compliance with applicable regulations and industry standards as needed for reimbursement.
Complete all documentation within timelines outlined by agency, departmental and regulatory standards.
Plan for the transition from Early Intervention to Preschool, to school based services (BOCES), and then the community; preparing the individual to function as independently as possible away from therapy service as targeted outcomes are achieved.
Evaluate, fit, adjust, fix and maintain prosthetic, orthotic and adaptive equipment devices and recommend client specific modifications necessary for function at home and/or school.
Provide direct supervision of physical therapy assistants in accordance with New York State Physical Therapy Practice Act
Participate with improving building accessibility and planning environmental modification.
Document need, advocate and provide recommendations in ordering/purchasing equipment for agency use.
Provide clinical educational opportunities for students of physical therapy.
Complete incident reports as indicated.
Perform or ensure that a daily safety inspection of your site is conducted to minimize and address any possible safety hazards and to ensure all equipment is being used safely and properly.

Supervisory Responsibility - May be required to provide service supervision for a Physical Therapy Assistant that is licensed by the State of New York if assigned
Travel - Travel is expected for this position. This could include county to county travel, home visits, school and/or county meetings, conferences and trainings.
Work Environment - This job operates in a professional office environment and therapy rooms. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
