Our facility in Ashland is just minutes away from the thriving downtown district. It is also centrally located between Mansfield, Medina, and Wooster. Crystal Care has an excellent therapy team that is committed to providing a variety of services to short-term rehab patients, outpatients, and long-term residents. The therapy gym is equipped with all the tools that a therapist needs to provide the most effective treatment interventions. Our dedicated therapy team has a proven track record of getting results!

Master's Degree in Speech Language Pathology from an accredited program Valid license to practice in Ohio.

POSITION DUTIES AND RESPONSIBILITIES:

1. Assigning patient care activities to professional and non-professional personnel.

2. Interpreting responsibilities and SNF policies to therapy personnel.

3. Assures the execution of doctor's orders.

4. Oversees the maintenance of the therapy records (medical, personnel, accounting, billing, etc.)

PRIMARY THERAPY RESPONSIBILITIES:

1. Evaluates and treats communication disorders, including motor speech disorders, language disorders, cognitive language deficits and voice disorders.

2. Provides direct patient services to improve the cognitive, communicative and or swallowing abilities of neurological impaired persons.

3. Evaluates, treats and provides case management for dysphagic patients.

4. Provides documentation to maintain quality of the medical records, as it relates to reimbursement guidelines, accuracy of billing information and facility requirements.

5. Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice.

6. Follows administrative policies and procedures related to emergency procedures, infection control and safety.

7. Coordinates and or participates in training programs for facility staff, residents, families and rehabilitation staff.

8. Initiates new program development with focus on functional outcomes to improve the quality of life of the people residing in the facility.

9. Participates in continuous quality improvement activities.

10. Performs other related duties as required.

Expected hours: 10 - 15 per week
