Hospice and Palliative Care Physician opening in OH Located in Findlay, 40m to Toledo Full Time Position Our Medical Group has a Hospice and Palliative Medicine Opportunity in Northwest Ohio.-Bridge Hospice & Palliative Medicine Service-Inpatient and community-based palliative medicine-Two inpatient hospice care centers, Cerner EMR-Available by phone or answering service on a 24-hour basis as an on-call consultant to clinicians within Bridge Home Health and Hospice-Expertise in symptom management of terminal illness-Lead provider based palliative medicine team in clinic, community and inpatient settings-Positive service-oriented interpersonal and communication skills-Knowledge and skills to mentor Hospice team in providing consistent quality patient care-Interact with the Hospice team for patient care planning-Participate in agency and community-based programs to educate medical professionals and lay persons in hospice and palliative medicine philosophy and services-Assist with the development, maintenance and review of patient care policies and proceduresMedical Group is a nonprofit, integrated regional health system based in Findlay, a unique micropolitan community and the second-largest city in Northwest Ohio.
