Job description

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN) or physician in a healthcare, long-term care, or assisted living facility. The LPN is responsible for monitoring patients health, administering basic nursing care, and ensuring that patients medical needs are met. This role is essential in promoting the health, comfort, and well-being of residents by delivering high-quality nursing care in a compassionate and professional manner.

Key Responsibilities:

Patient Care:

Provide basic nursing care to residents, including administering medications, wound care, and assisting with personal hygiene.
Monitor residents' vital signs, such as temperature, blood pressure, and pulse, and report any abnormalities to the supervising nurse or physician.
Assist with feeding, dressing, and mobility, ensuring residents comfort and safety.
Follow individualized care plans and provide care according to residents needs and medical conditions.

Medication Administration:

Administer medications to residents as prescribed by physicians, ensuring accurate documentation and timely delivery.
Monitor residents for adverse reactions to medications and report any concerns to the supervising nurse or physician.
Maintain accurate records of medication administration and residents responses to treatments.

Documentation and Reporting:

Maintain up-to-date and accurate documentation of patient care, including vital signs, treatments, and any changes in condition.
Report residents progress, concerns, or changes in health status to the supervising nurse or physician.
Ensure that all documentation complies with facility policies and healthcare regulations.

Collaboration and Communication:

Work closely with RNs, physicians, and other healthcare professionals to ensure cohesive patient care and effective communication.
Communicate with residents and their families, addressing concerns and providing updates on residents' health and treatment.
Participate in care conferences and interdisciplinary team meetings to discuss resident care plans and outcomes.

Infection Control and Safety:

Follow infection control protocols and maintain a clean and safe environment for residents, staff, and visitors.
Use personal protective equipment (PPE) appropriately and ensure compliance with facility safety guidelines.
Report any safety hazards or concerns to the appropriate personnel.

Resident Advocacy:

Act as an advocate for residents, ensuring their needs are met and their dignity is respected.
Provide emotional support to residents and their families, offering comfort and reassurance when needed.
Promote a positive and compassionate environment for all residents, fostering their physical and emotional well-being.

Education: Completion of an accredited LPN or LVN program.
Licensure: Current and active Licensed Practical Nurse (LPN) license.
Experience: Previous experience in a healthcare or long-term care setting is preferred but not required. New graduates are welcome to apply.
