Job description
Qualifications: Bachelor of Science Degree in Physical Therapy from and accredited program. Successful completion of National Certification Examination for Registered Physical Therapist. Assumes responsibility for ongoing continuing education and professional development. Current state licensure or license eligible.
As a Licensed Physical Therapist in a Grace Management Services facility you must be able to perform the following
essential functions as demonstrated by the ability to:
Essential Job Functions:
Provides physical therapy services to residents and initiates referrals when appropriate and in accordance with Physical
Therapy Department Procedures.
Conducts screenings of residents at regular intervals to determine need for intervention/treatment.
Evaluates residents to obtain data necessary for treatment planning and implementation and conducts specialized
evaluations as indicated.
Interprets and communicates evaluation findings to residents, families and team members.
Establishes individual resident schedules in conjunction with other team members.
Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
Coordinates treatment plan with resident, family and other team members.
Implements and conducts treatment as outlined in treatment plan.
Monitors residents response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
Documents results of residents assessment, treatment, follow-up and termination of services.
Adheres to department and resident schedule and modifies as appropriate for treatment regimen.
Maintains acceptable resident care ratios as determined by the Physical Therapist.
Adapts treatment to meet the individual needs of the resident.
Provides, recommends and fabricates adaptive devices or other equipment. Trains resident, family and appropriate staff
in the use or application of such equipment.
Participates in discharge planning.
Supervises and directs Licensed Physical Therapy Assistants (LPTA) in the implementation of the treatment plan.
Supervises Rehabilitation Aides, Physical Therapy Aides, students and volunteers.
Reviews the quality and appropriateness of the total services delivered and individual physical therapy programs for
effectiveness and efficiency, using predetermined criteria.
Participates in resident, family and staff education.
Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
Adheres to Physical Therapy Department procedures regarding documentation of physical therapy services.
Provides in-service education to the members of the residents care team and education to the community.
Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
Assists with and participates in Quality Assurance Program as required.
Maintains a current knowledge of the law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.
Attends and participates in scheduled training, educational classes and meetings required or requested for position.
Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions and OSHA standards for the healthcare professional.
Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
Responds to and acts appropriately in emergency or disaster situations.
Adheres to high standards of cleanliness, grooming, hygiene and dress code.
Adheres to all policies of company, including job description, certification requirements, mission statement, HIPAA, confidentiality standards and residents rights.
Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and in flexible in carrying out assignments.
Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control.
Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
