We (Torre) are helping Physician's Choice Home Health Care find a top candidate to join their team full-time for the role of Registered Nurse Case Manager.


Compensation:

USD 100K - 115K/year.


Location:

Beaverton, OR, USA.

Responsibilities and more:

Physician's Choice Home Health Care seeks a compassionate and highly motivated Registered Nurse Case Manager to join our dedicated team. As a Registered Nurse Case Manager, you will play a crucial role in providing exceptional care and support to our patients in the comfort of their own homes. Responsibilities:

- Conduct comprehensive assessments of patients' health status and individualized needs to develop personalized care plans.

- Coordinate and manage patient care in collaboration with physicians, therapists, and other healthcare professionals.

- Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.

- Monitor patients' progress and update care plans accordingly, ensuring interventions align with their evolving needs.

- Educate patients and their families on disease management, self-care techniques, and available community resources.

- Advocate for patients' rights and ensure their preferences and goals are integrated into the care plan.

- Maintain accurate and up-to-date documentation of patient assessments, care provided, and outcomes achieved.

- Collaborate with interdisciplinary teams to facilitate seamless transitions of care and improve overall patient well-being.

- Adhere to regulatory guidelines, company policies, and industry best practices.


Qualifications:

- Valid Registered Nurse (RN) license in Oregon.

- Minimum two years of nursing experience, preferably in home health or a related field.

- Strong clinical assessment and critical thinking skills.

- Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.

- Ability to prioritize and manage multiple cases efficiently.

- Demonstrated compassion, empathy, and sensitivity in providing patient-centered care.

- Knowledge of Medicare guidelines and home health regulations is desirable.

- Proficiency in electronic health record (EHR) systems and basic computer skills.


Important:

- Schedule: On call.

- License/Certification: RN (Preferred).

- Willingness to travel: 75% (Preferred).

- Work Location: On the road.

- Job Types: Full-time, Part-time.
