The National Health Service Core (NHSC) Loan Repayment Program is open to licensed primary care medical, dental, and mental and behavioral health providers who are employed by accredited sites.

Transitional Housing Specialist Pay Range:
Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by your education and experience as required by the role, as well as internal equity and alignment with market data. Typically, new team members join at the minimum to mid salary range.
Salary: $16.60 - $20.75 DOE

Transitional Housing Specialist Summary:
Transitional Housing Specialists (THS) work closely with CBCS therapists, psychiatric providers, community partners, and medical providers. This position involves participation and implementation of program goals and objectives to promote a strengths-based, wellness-informed client centered approach to assisting individuals in achieving consumer-driven goals.
Transitional Housing Specialist Education: Must meet Oregon State (OARS) requirements and MHACBO certification as a "Qualified Mental Health Associate (QMHA)," meeting one of the following qualifications:
- Bachelor's degree in a behavioral science field
- A combination of at least three years relevant work, education, training, or experience.
Transitional Housing Specialist Experience: Experience to demonstrate the competencies to identify precipitating events:
- Ability to communicate effectively
- understand mental health assessment, treatment, and service terminology and apply each of these concepts.
- Implement skills development strategies, and identify, implement, and coordinate the services and supports identified in an ISSP.
- Preferred experience: Previous experience working with adults diagnosed with mental illness or any disability group preferred
Transitional Housing Specialist Responsibilities:
- Responsible for meeting client engagement, outreach and follow-up goals as established by contract guidelines and KBBH goals and targets.
- Responsible for tracking and documenting referral disposition, including following up with the client. Documentation includes data collection, electronic health record (EHR) documentation, response letters, and requests for records.
- Participate in team treatment and coordination, providing critical information to the team as well as executing referral and navigation recommendations from the team. Provide case management and skills building services, in accordance with the treatment plan and/or Transition Plan.
- Provide group treatment and skills building services in accordance with the treatment plan and/or Transition Plan.
- Assists families/clients in transition for further care; assists in locating alternative care sources.
- Complies with federal, state, and local legal confidentiality requirements, including HIPAA and Oregon Administrative Rules.
- Identifies and assists clients in reducing barriers to care. Communicates in an understandable manner and monitors clients' understanding.
- Enhances personal growth and development through attendance and participation in meetings and conferences.
- Serves as support position for the CBCSTH Team Lead and CBCS/CBCSTH Supervisor.
Certificates, Licenses, Registrations: Valid Oregon drivers license, insurable driving record. Personal automobile insurance is required if clients are transported, or any agency business is conducted by the employee using his/her personal vehicle. Must meet the requirements to become MHACBO QMHA Certified. Certification must be maintained for the length of employment.
