Job description
The practitioner works with the Medical Director to provide medical care to patients admitted to 16th & Burnside Recovery Center & Hooper Detox Center. A strong background in mental health and addiction medicine is preferred, as well as experience working with homeless and underserved populations. Demonstrated skills in addiction and/or primary care medicine, graduation from an accredited medical school, unrestricted licensure to practice medicine in Oregon, and to have proper training prescribing Buprenorphine are required of all applicants.
A strong background in mental health and addiction medicine and the ability to meet the training requirements for buprenorphine prescribing are strongly preferred. Experience working with homeless and underserved populations is strongly preferred. Successful applicants will understand the value of harm reduction and low-barrier buprenorphine prescribing and will be flexible and responsive to highly vulnerable patients. Demonstrated skills in primary care medicine and licensure to practice medicine in Oregon are required of all applicants.

Schedule: Monday-Friday 8:00am-5:00pm

MINIMUM QUALIFICATIONS: NP/PA
Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner applicants.
If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or Licensed to practice as a Physician Assistant in Oregon, with at least 2 years experience.
MINIMUM QUALIFICATIONS: MD/DO/ND
Board eligibility or certification in Internal Medicine or Family Practice.
Graduation from a CNME-accredited, four-year Naturopathic Medical program.
Must have completed a CNME-certified primary care residency program.
Licensed naturopathic physician in the State of Oregon.
Maintain an Active DEA Certification.
MINIMUM QUALIFICATIONS: ALL APPLICANTS

Demonstrated skills in primary care medicine with a minimum of 2 years' experience preferred.
Appropriate training to prescribe Buprenorphine.

Must meet CCC privileging requirements as required by FTCA.
Available to work during standard Hooper Detox Center and 16XBurnside Recovery Center hours.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Familiarity with standing orders for 16th & Burnside & HDSC.
Provides admission orders for all new patients, including orders for the administration of Buprenorphine.
Conduct comprehensive health assessments, including detailed health histories and thorough physical exams for all newly admitted patients.
Provide urgent and primary care services to high-risk, vulnerable populations, particularly individuals experiencing homelessness, ensuring compassionate and responsive healthcare tailored to their unique needs.
Orders, interprets, and evaluates diagnostic tests to identify and assess patients' clinical problems.
Formulates stabilization or care plans based on available clinical data and works cooperatively with patients to implement treatment plans.
Conducts on-going health care and clinical management of stable, chronically ill patients.
Determines and manages minor and/or acute illnesses and initiates treatment.
Conducts initial care for emergency situations such as illness or injury and initiates arrangements for continuing care.
Collaborates with other health care team personnel and case management personnel about patient treatment.
Designs and oversees personalized wound care treatment plans aimed at promoting optimal healing or managing chronic conditions, ensuring the highest standard of care and patient well-being
Prescribes medication or other forms of treatment, as necessary.
Keeps appropriate records.
Participates in compliance, qua15. Is available to take calls from the treatment team even when off-site when working.

Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
Required to participate in the after hours on-call pool.
