Licensed Practical Nurse (LPN) - Full time

The Role:

The comfort, compassion, teaching, and empowerment you provide as a LPN can help hospice patients and their families make the most of their time left together.By providing expert, personalized care to hospice patients where they live you will bring dignity and compassion to those coping with symptoms of a serious illness.

Who You Are:

Requirements:
Licensed Practical Nurse with a current license to practice in Oregon
One year of hospice or home health nursing experience preferred.
Working knowledge of and the ability to apply the basic principles and techniques of nursing care to the hospice setting.
Must possess good clinical nursing skills and be able to utilize problem-solving teachings.
Ability to record and maintain patient data using an electronic record keeping system (EMR).
Knowledge of pain and symptom management.
Ability to function in high stress situations and manage multiple priorities.
Emotional maturity to cope with the ongoing needs of participating in a hospice program.
Ability to regularly walk bend, stoop, kneel, push, pull, grasp, reach, handle, and lift up to 50lbs without assistance.
Current CPR certification required.
Must have current Oregon driver's license, dependable transportation, and proof of current automobile insurance.

Responsibilities:
Provide direct nursing care as indicated by the plan of care under the supervision of RNCM
Participate in the planning and coordination of patient care
Administers prescribed meds according to law as permitted within the scope of individual license
Provide patient/caregiver teaching as appropriate
Provide physical and emotional support to the patient/family in the terminal phases of life
Demonstrate respect and caring and maintains patient and staff confidentiality
Assess patient/family general physical and mental condition with the assigned RN Case Manager
Assists with provision of in- home respite and/or continuous care as patient need arises
Assist patient with activities of daily living and encouraging appropriate self-care
Documents according to established agency guidelines and nursing practice standards
Instruct family or caregivers in caring for the patient as directed
Communicates in an effective, accurate and respectful manner
