Job Description Mon Yough Community Services is seeking to hire multiple caring and dedicated employees at various locations!

The Direct Support Professional will support and provide direct care to intellectually disabled adults within a 24-hour supervised Community Living Arrangement program. The employee in this role must ensure the safety and well-being of the residents throughout his/her shift while treating individuals with respect, dignity and provide opportunities to grow and develop everyday life skills that promote independence and community integration. Will be responsible for maintaining HIPAA and Code of Federal Regulations Chapter 42 Public Health confidentiality standards.

Individuals in the program face everyday challenges and opportunities with the support of dedicated and trained staff. On most days, consumers are transported to work or a day program they have been scheduled to attend. The consumers are responsible for planning and cooking their own meals, but receive assistance in budgeting, medication administration, socializing, attending doctors visits and participating in their communities. Intellectually and/or Developmentally Disabled individuals receive assistance as indicated in their Individualized Support Plan.

Shifts will include daylight, evening/night, overnights and variable shifts. All shifts will likely include weekends. Flexibility is key as this is a residential program that may require overtime beyond scheduled shifts.

Hires for this position must have a High School diploma or GED, as well as a valid driver's license. Prior human service or caregiving experience is preferred, but it not required.

Responsibilities:
  • Active participant in recreational and leisure activities by planning, organizing and/or attending with transportation plan identified.
  • Working knowledge of all resident files including medical, central, programming, goal plans, support Supports, etc.
  • Obtain 24 hours of staff development per year in accordance with our program policy and procedures, as well as, state and county regulations.
  • Responsible for assisting residents to eat well-balanced diets and/or follow specific doctor-ordered diets.
  • Accompany and assist residents on all appointments.
  • Responsible for personally reporting yourself off to your supervisor or the designated supervisor on-call at least four hours prior to your shift.
  • Assist residents in all aspects of teaching and/or completing with residents all adult daily living skills, including personal hygiene, bathing, toileting, oral hygiene, dressing, cooking, cleaning, shopping, running errands, transportation, packing lunches, laundry, etc.
  • Remain calm, cooperative, and supportive while on duty, especially in emergency and unsettling circumstances.
  • Responsible for ongoing monitoring to know where residents are located and what they are doing to ensure residents' safety and well-being when home and in the community including bed checks if indicated.
  • Complete regular household cleaning chores including, but not limited to: defrosting the refrigerator, stove/oven cleaning, curtains/drapes, windows, marks on walls, bathroom cleaning, etc.
  • Maintain awareness of current ISP for individuals assigned and agrees to adhere to the content of the ISP in delivery of Supports and supports.
  • Medication administration as mandated and trained under the Department of Public Welfare for staff who work in Pennsylvania's community-based ID programs.
  • Provide support and guidance to residents for their emotional, social and intellectual growth.
  • Cash box responsibility as assigned. File outdated staff notes, progress notes and other paperwork as directed. Inventory and reorder all paperwork forms and supplies. Attends meetings as required. Assists other programs as directed.
  • Be able to perform computer tasks within several software applications including but not limited to Microsoft Office, PsychConsult and payroll system.
  • Monitor the residents living area for hazardous and routine maintenance conditions and the reporting of such conditions upline.
  • Report to work with regular attendance and be able to stay awake throughout your shift as well as not leave residents unattended until a staff person arrives to relieve you of duty.
  • Operate an automobile or van (including lift accessories) as appropriate to provide client transportation.
  • Assist with the development, implementation and documentation of the individual client program issues, such as the ISP, assessment, outcomes, monthly summary, etc.
  • Work independently, under stress and change while performing multiple tasks simultaneously. Cannot impose personal beliefs and practices on others, as well as keeping personal matters out of the work environment.
  • Provide quality Supports at all times to residents regardless of medical and psychiatric behavioral issues. Supports may include CPR, first aid, behavioral programming, goal plans, etc.
  • Ensure the residents' environment is continuously clean, safe and secure.
  • Basic computer knowledge required or the willingness to learn. Assist individual with banking, spending money, bill paying, recreation money, bus fare, laundry money, as well as assisting with grocery shopping and the calculation of the individual's grocery money. Budgeting individual funds to pay monthly bills monitor spending money, filling out necessary forms.
  • Completion of all paperwork, as required, in a timely fashion with signature and date. Paperwork includes progress notes, staff notes, goal plans, appointment sheets, time sheets, expense sheets, etc.
  • High school diploma or GED required.
  • Prior human services experience preferred.
  • Must have a valid PA driver's license and meet the guidelines to be accepted on the agencies vehicle insurance policy.
  • Upon hire, provide a current physical and Mantoux test, and then ongoing as required by the Department of Public Welfare (DPW).
Licensure, Certifications, and Clearances:
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 34
