Diabetes Educator - Endocrinology

Location: Philadelphia, PA

Virtus Healthcare Staffing is seeking a dedicated and passionate Diabetes Educator to join our partner Medical Group in Philadelphia, PA. This is an essential role within the Endocrinology department, focused on empowering patients with diabetes to manage their condition effectively and achieve optimal health outcomes.

Responsibilities:

Patient Education: Provide comprehensive diabetes education to patients and their families, covering topics such as blood glucose monitoring, medication management, nutrition, exercise, and self-care strategies.

Individualized Care Plans: Develop and implement personalized care plans tailored to each patient's specific needs and goals.

Support and Empowerment: Offer ongoing support and guidance to patients, helping them navigate the challenges of living with diabetes and achieve a healthy lifestyle.

Collaboration: Work collaboratively with the endocrinology team, including physicians, nurses, and other healthcare professionals, to provide coordinated and comprehensive care.

Program Development: Participate in the development and implementation of diabetes education programs and initiatives within the medical group.

Documentation: Maintain accurate and detailed patient records, ensuring compliance with all relevant regulations.

Qualifications:

Education: Bachelor of Science in Nursing (BSN) OR Non-Nursing Bachelor's degree with an MSN OR Doctoral degree in Nursing

Experience: Previous experience as a Diabetes Educator is required.

Licensure/Certification:

Current PA Registered Nurse License

BLS Certification

CDE (Certified Diabetes Educator) or CDCES (Certified Diabetes Care and Education Specialist) Certification (Required within 12 months of hire)
