General Responsibilities and Accountability

Assesses patient's physical therapy needs and assist the physician in evaluating level of function.

Develops and implements plans of care appropriate to the assessment findings.

Provides physical therapy to patients in the home or community setting.

Teaches and instructs patients regarding active and passive exercises.

Treats patients to relieve pain and/or develop or restore function to maintain maximum performance.

Evaluates patients for adaptive equipment needs and obtains physicians orders for same.

Arranges for necessary adaptive equipment.

Educates patients and caregivers in the use of assistive devices.

Documents all patient care on appropriate forms/patient's medical records.

Submit completed documentation within established time parameters.

Serves as a resource to the management and field staff of the organization.

Participates in multidisciplinary case conferences and staff meetings.

Demonstrates a high degree of commitment to customer service and the provision of quality care.

Supervises the Physical Therapy Assistant and Home Health Aide when appropriate to the patient's care provision.

Participates in the utilization review and quality improvement activities of the organization.

Reviews and recommends physical therapy policies and procedures for the organization which are in compliance with Medicare, federal and state credentialing criteria and standards.

Completes other tasks as requested or assigned.

Education and Licensure

Degree Requirement: Graduated from a physical therapy program accredited by the Pennsylvania Commission on Accreditation in Physical Therapy Education or by another National organization recognized by the Board that accredits physical therapy programs.

License Requirement: Active license through the State of Pennsylvania.

Experience Requirement: A minimum of one (1) year of Institutional Physical Therapy experience.

Experience Preferred : Previous home healthcare experience welcome, but not required.

Competency and Skill Requirements

Excellent written and verbal communication skills. Able to utilize the telephone for internal and external communications. Knowledgeable of therapy assessment skills. Competent in the provision of therapy care.
