Job Overview - Psychiatric Field Nurse (RN):
Join our client as a Psychiatric Field Nurse (RN) in Philadelphia, PA, providing community-based care to individuals with severe mental illness. This field-based role, requiring travel across Philadelphia County, allows you to make a direct impact by coordinating behavioral and physical health services for homeless individuals.

Compensation: $52/hour
Location: Philadelphia, PA
Schedule: Monday to Friday, 9:00 AM to 5:00 PM (Full-time)
Setting: Fieldwork across Philadelphia County


Responsibilities as the Psychiatric Field Nurse (RN):

  • Care Coordination: Oversee the behavioral and physical health needs of service participants, ensuring that care aligns with physician orders and psychiatric nursing practices.
  • Medication Monitoring: Manage participants' medications, providing psychiatric nursing services and ensuring compliance with treatment plans.
  • Community-Based Care: Conduct health and biopsychosocial assessments in the field, with approximately 75% of work in community settings.
  • Collaborative Care: Work as part of a multidisciplinary Assertive Community Treatment (ACT) team to help participants reach their maximum level of functioning in the community.
  • Consultation: Collaborate with team members on participant care plans, coordinating routine medical, vision, lab, and dental appointments.
  • Documentation: Maintain accurate and timely records of medical histories, treatments, and assessments, ensuring compliance with agency and state requirements.
Qualifications for the Psychiatric Field Nurse (RN):
  • Licensure: Board Certified Registered Nurse (RN), licensed in Pennsylvania.
  • Experience: Experience providing psychiatric services in community mental health settings.
  • Knowledge: Expertise in psychiatric care and the ACT model, with strong clinical knowledge.
  • Skills: Excellent organizational, communication, and analytical abilities.
  • Other: Valid driver s license with a clean driving record required.
