This position is remote or at a local office on Mondays and Fridays and then Tuesday, Wednesday, and Thursday are in the field in the Pittsburgh, PA, Morgantown, WV, and Ohio border areas.

Candidate will reside around the Pittsburgh, PA or Morgantown, WV areas.

As a Performance Improvement Manager Home Health RN , you will be responsible for coordinating the development, monitoring and management of the Performance Improvement Program (PIP) of the offices. Advises the offices on Performance Improvement Plans. Responsible for education and guidance to offices on clinical, compliance, and performance improvement issues, in accordance with applicable law, regulation, and appropriate standards of practice. The Performance Improvement Manager will assist offices to achieve performance improvement objectives utilizing benchmarking data.

Communication:

Communicate in a positive, professional, and respectful manner, at all times, verbally and nonverbally.

Maintain open communication with the administrators/managers and clinical leaders of the offices.

Collaborate with Corporate Clinical Services Team to identify company-wide areas for improvement, and educational needs.

Report concerns/audit results/office visit findings to appropriate people (administrators, RVPs, CCO, COO, President)

Communicate weekly, in advance, to direct supervisor schedule, project work, office specific plans, upcoming audits, office visits, etc

Quality, Compliance and Clinical Operations:

Advise assigned offices on the Performance Improvement projects. Review prior to submission to Governing Body. Ensure plans are followed, and quarterly updates occur.

Provide oversite of the QAPI program for assigned offices.

Collect and analyze outcome data to develop education and guide assigned office's Quality Assurance & Performance Improvement (QAPI) initiatives in collaboration with the office's clinical management team.

Support and guide the ongoing performance improvement efforts of the offices to improve star ratings for patient outcomes of care and patient experience.

Perform clinical records audits based on identified need and/or by individual performance improvement initiatives of the offices.

Employee Retention:

Participate with onboarding program for new clinical managers, as requested.

Utilize a collaborative approach when interacting with office staff; encourage open dialogue regarding agency issues to improve overall compliance with expected standards.

To qualify for a Performance Improvement Manager Home Health RN position with us, you will need:

A Registered Nurse with a license in good standing in at least one state in the assigned region; bachelor's degree preferred.

Three (3) years of experience as a nurse and one (1) year of experience in home health preferred.

One (1) year of experience with and/or knowledge of performance improvement activities is preferred.

Good knowledge of home health Medicare Conditions of Participation and state licensure regulations, where applicable.
