Job description
Position Summary:
This position is offering a $25,000 signing bonus! As a member of the interdisciplinary team, the Hospice Nurse works under the general direction of the Director of Hospice and the attending physician in the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible to provide direct supervision of Home Health Aides and or LPN's providing care for Guthrie Hospice.
Education, License & Cert:
Registered Professional Nurse currently licensed in Pennsylvania. Knowledge of nursing theory and practice at a level of normally associated with an Associate's Degree. Education and experience requirements may vary according to combined background of the individual.
Experience:
One (1) year nursing experience in an acute care setting and/or previous Hospice experience or similar work with patients/families experiencing terminal illness is preferred. Experience and skill in supervision desired.
Essential Functions:
1. Congruent with Guthrie Hospice Nursing Model, assume primary responsibility of at least a one to fifteen patient /family caseload that includes the accessing, planning, implementing, and evaluating phases of the nursing process.
2. Initiate communication of attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
3. Maintain regular communication with Hospice director, Medical Director and attending physician concerning caseload.
4. Obtain data on physical, psychological, social and spiritual factors that may influence patient/family status and incorporate that data into the plan of care.
5. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
6. Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
7. Accept responsibility for coordinating physical care of the patient by teaching of the patient by teaching primary caregivers, volunteers, employed caregivers (LPN's, hospice aides), or by providing direct care as appropriate.
8. Meet regularly with hospice nursing staff, and attend interdisciplinary team meetings to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth, and facilitate issues from caseload for full staff discussion, consultation and evaluation.
9. Supervise and document hospice aide plan of care every 2 weeks and supervise and document the practice of the LPN in the provision of patient care as delegated.

2.Travel is necessary for this position.
3. On Call is required.
